One-Time Consultation

Families often have one specific concern, but they may not be certain about next steps. We meet with families on a one-time basis to discuss strategies, provide resources and clearly define next steps.

Comprehensive Assessment

If family members are unsure how to help an older adult, a thorough geriatric assessment is often the best way to start. We will assess physical safety, nutrition, cognition, medication management, social wellness, legal and financial issues, as well as screen for any depression or anxiety that may be present. From there, we can address any immediate needs and recommend a long-term plan.

Our assessment includes:

Simple modifications to the home can reduce the likelihood of an unnecessary fall. So, we review the physical environment of our clients and make recommendations to increase safety and ease of mobility.

Our care managers will establish a baseline for cognition during the initial visit in order to monitor changes over time and address them as needed.

We assess an older adult’s support network and evaluate how often they engage with others. We can recommend social and cultural activities, as well as provide transportation, if that is an obstacle.

We review our client’s medical history in order to understand how medical limitations impact daily living. We keep copies of current medical conditions, medications and recent procedures in case of emergency.

Our team serves as an advocate to assist you or your loved one with financial and legal issues, which may include:

End of Life Documentation
It’s important to ensure that the client’s wishes for end-of-life are known. When advanced directives and information regarding durable medical power of attorney are provided, we can suggest updates and provide necessary documents in the event of an emergency.

Guardianship & Conservatorship
Many older adults fall victim to scams or financial exploitation. Appointing a conservator or guardian can protect the older adult as well as alleviate any stress for family members.

Ongoing Case Management Services

The needs of an older adult change over time and family members can’t always be there to observe what can often be subtle shifts. But through monthly visits that feel largely social, we are tuned in to any changes in your loved one and are always assessing if services need to be adjusted. Should more intensive support be needed, we can accompany clients to doctor appointments, check in on them in a hospital or rehab setting, and provide advocacy and insight to medical professionals regarding changes in their physical or cognitive condition.
